Procedure for Responding to and Documenting Medical Emergencies related to Animal Care/Research Worksite Injuries

1. Notify supervisor and LAF director (829-2919).
2. Call Annette Lozo, Workers Compensation Administrator, 120 Crofts Hall, 645-5000, x 1025.
3. During work hours (8-4, M-F) call Occupational Medicine at ECMC to inform them that you need to see a doctor. Phone number is 898-4153. Proceed to Occupational Medicine Department at ECMC, 462 Grider Street, Buffalo.
4. After hours, proceed to ECMC emergency entrance and tell the front desk that you are enrolled in the Occupational Heath and Safety program at UB and you were hurt at work.
5. Cost for injury/emergency care is covered under Worker's Compensation for employees, or Student Health Insurance for students.
